Our role in post-discharge care
Coordination with the Discharge Team
Our occupational therapists collaborate with the hospital's discharge team to make sure all medical, functional, and safety needs are addressed before the patient returns home. This includes assessing daily living requirements and ensuring the right supports are in place.
Supporting Families
Families often experience stress and uncertainty after discharge. Our OTs provide guidance and training to caregivers, helping them understand how to assist safely while avoiding burnout. This hands-on support helps families feel confident in managing daily routines.
Assistive Devices and Approvals
OTs help identify which assistive devices are needed, such as mobility aids, adaptive equipment, or home modifications, and prepare recommendations for insurer approval. They follow up to ensure equipment is delivered and properly installed.
Personal Care and Independence
OTs work closely with patients on strategies for personal care — dressing, bathing, and mobility, tailoring approaches to maximize independence. This not only supports physical recovery but also reduces the frustration that many injured patients experience during the transition back home.
