In-home Assessment involves a visit by a healthcare professional, usually an occupational therapist or registered nurse, to evaluate a patient’s functional status, safety, and care needs within their home setting. This personalized assessment allows for a better understanding of daily challenges and living conditions to support recovery.

The assessment includes a review of medical history, physical and cognitive abilities, mobility, self-care capacity, and home safety. It may also assess the need for home support services, assistive devices, or caregiver assistance.
When is an In-Home Assessment needed?
This assessment is recommended for patients who:

Have limited mobility or chronic illness,

Recently returned home after hospitalization or rehabilitation,

Are recovering from a motor vehicle or workplace injury,

Are at risk of falls or safety issues at home,
