
Home Modification Assessment
Home Modification Assessment involves an in-depth evaluation of a patient’s living space to determine what changes are needed to enhance safety, accessibility, and function. Conducted by an occupational therapist, this assessment includes a home visit, physical assessment of the patient, and analysis of barriers within the home environment.

The clinician evaluates entranceways, hallways, bathrooms, kitchens, stairs, bedrooms, and other areas regarding the patient’s limitations and injuries. Recommendations may include grab bars, ramps, stairlifts, widened doorways, non-slip flooring, or changes to furniture placement.
When is a Home Modification Assessment needed?
This assessment is essential for patients who:
