Activity of Daily Living Assessment

An Activity of Daily Living (ADL) Assessment evaluates a patient’s ability to perform everyday tasks essential for independent living. These include basic self-care activities like dressing, bathing, eating, toileting, grooming, and mobility. The assessment is typically conducted by an occupational therapist.

The clinician observes the patient’s performance, identifies barriers, and may use standardized tools to measure function. The assessment can take place in a clinic or at home. A detailed report is generated to guide rehabilitation goals, home care services, or documentation for any patient's needs.

When is an ADL Assessment needed?

ADL Assessments are often recommended for individuals who:

  • Are recovering from physical or neurological injuries,

  • Experience cognitive decline or memory impairment,

  • Struggle with self-care after an accident or surgery,

  • Are managing chronic conditions like arthritis, stroke, or dementia,

  • Need functional evaluations for legal, insurance, or long-term care planning.
